Support that continues long after treatment ends
Recovery is not measured by the day someone completes treatment. It is measured by what happens in the weeks, months, and years that follow. Treatment is only one part of a much larger recovery journey, and as clients return to work, family responsibilities, school, and everyday life, they begin applying the skills they built in treatment to real-world situations. That transition can be exciting, but it can also be one of the most vulnerable periods in early recovery.
Our Aftercare & Alumni Program is designed to help clients stay connected, maintain accountability, and continue building the healthy lives they worked so hard to create during treatment.
Why aftercare matters
Recovery does not happen all at once. Life continues after treatment: stress returns, relationships need rebuilding, careers demand attention, family responsibilities grow, and unexpected challenges show up. Without ongoing support, even the strongest foundation can start to feel unstable. Staying connected to recovery professionals, peers, healthy routines, and a supportive community provides the encouragement and accountability needed to navigate life's inevitable ups and downs. Our goal is for every client to leave treatment with confidence, and with the support to keep moving forward.

Learn moreWhat aftercare may include
Every continuing care plan is individualized based on each client's needs, goals, and stage of recovery. Depending on the situation, aftercare may include:
- An individualized continuing-care plan
- Alumni community participation
- Ongoing recovery check-ins
- Referrals for individual therapy or psychiatry
- Recovery meeting recommendations
- Relapse prevention planning
- Family support resources
- Community referrals and additional recovery services
Recovery is not about following a single path. It is about finding the combination of support that helps each person continue growing.
Staying connected through our alumni community
One of the greatest strengths of recovery is community. Our alumni program helps former clients stay connected with peers who understand the challenges and rewards of recovery. Whether it is attending alumni events, celebrating recovery milestones, reconnecting with people they went through treatment with, or simply knowing there are familiar faces to reach out to, those relationships often become an important part of long-term success. Participation is always voluntary, but recovery becomes stronger when people remain connected.
Preparing for real life
Treatment provides a safe environment to begin healing. Aftercare helps clients apply those lessons once everyday life resumes. Returning to work, rebuilding trust with family, managing stress, developing healthier routines, and maintaining recovery outside a structured treatment environment all take practice and ongoing support. Our clinical team begins preparing clients for this transition throughout treatment, so they leave with realistic expectations, practical tools, and a clear plan for continued success.
If recovery becomes difficult
Recovery is rarely a perfectly straight line. Most people experience periods of uncertainty, stress, or emotional difficulty along the way. Those moments do not mean someone has failed. They are often opportunities to strengthen recovery with additional support. We encourage former clients to reach out early whenever they feel themselves struggling. Sometimes a conversation, an adjustment to the recovery plan, or reconnecting with supportive people makes all the difference.
If someone relapses
We believe relapse should be approached with honesty, compassion, and hope, not shame. For some individuals, relapse becomes part of the recovery journey. Rather than viewing it as failure, we see it as an opportunity to better understand what happened, identify what support is needed, and strengthen the recovery plan moving forward.
“Our philosophy is that, for some, relapse is part of their recovery journey. This is not a failure or even a choice. When our clients experience a relapse, the greatest compliment to us is that they don't feel ashamed to reach out and come back in, to pick up where they left off, address what worked and what didn't, and develop next steps.”
Recovery is a lifelong journey
Our commitment to clients extends beyond the completion of treatment. We hope every graduate leaves Present Moments Recovery with more than sobriety: healthier relationships, stronger coping skills, meaningful connections, renewed confidence, and a clear plan for continued growth. Recovery is built one day at a time, and we are honored to remain part of that journey.

Frequently asked questions
What is aftercare?
Aftercare is the ongoing support and recovery planning that continues after formal treatment ends. It helps clients maintain momentum as they transition back into everyday life, through continuing-care planning, alumni community, check-ins, and referrals when more support is needed.
Why is aftercare important?
The weeks and months following treatment can be one of the most vulnerable stages of recovery. Continuing support helps people stay connected, accountable, and focused on long-term recovery while real life resumes.
How long does aftercare last?
Every recovery journey is different. Continuing-care recommendations are individualized based on each client's needs, goals, and progress. Some people want structured support for months; others mainly want to know the door stays open.
What happens if I relapse?
Reach out as soon as you begin struggling. Relapse is approached with compassion and clinical support, not judgment, and we work together to determine the most appropriate next steps, whether that is an adjustment to your plan or a return to a higher level of care.
Can my family continue to receive support?
Yes. Family involvement often continues after treatment through education, counseling referrals, and ongoing communication whenever clinically appropriate.
Do I have to participate in the alumni program?
Participation is always voluntary, but many graduates find ongoing connection with peers to be an important part of maintaining long-term recovery.
Should I attend recovery meetings after treatment?
Many clients benefit from community-based recovery meetings as part of their continuing-care plan. Our clinical team can help determine what level of ongoing support is right for you.
How do I stay connected with Present Moments Recovery?
Participate in alumni activities, maintain contact with the treatment team when appropriate, and reach out whenever you need guidance, encouragement, or additional support. Call or text (619) 363-4767.
