Does insurance cover rehab?

Some health plans include benefits for substance use treatment. Whether a plan may pay depends on its benefits, network rules, medical requirements, and approval of the recommended care.

Federal law generally says that plans offering mental health or substance use benefits cannot place stricter limits on those benefits than they place on medical care. This does not mean every plan covers every treatment program. Insurance benefits and approval still need to be checked.

Commercial insurance plans we work with

PMR's commercial in-network payer relationships are listed below. Individual benefits and authorization still depend on the member's plan and recommended care.

Military Coverage

TRICARE and TriWest

PMR is an in-network provider for TRICARE and TriWest. Eligibility, benefits, authorization requirements, and clinical criteria vary by plan and must be verified before admission. Coverage, authorization, admission, placement, and payment are not guaranteed.

PPO and POS plans with out-of-network benefits

PMR can work with PPO and POS plans when the plan includes out-of-network benefits and the carrier authorizes the recommended care.

Some PPO and POS plans allow care outside their usual provider network, often at a higher cost to the member. PMR can work with these plans when out-of-network benefits are included and the insurance company approves the recommended care. Aetna is one possible carrier example, not the only one.
